This is a simple and effective tool to compliment your Electrical Safety Program. The example below illustrates a two sided card, approximately the same size of a business card. The simple Test-Before-Touch rules of:

are on one side, and you can add your logo to the back of the card, some of your own custom text (or multiple different messages on different cards) to emphasize key elements in your company's specific Electrical Safety Program, or you can include the following text on the back of the tag:
My Electrical Safety Principles
1. Plan Every Job
2. Identify the Hazards
3. Anticipate Unexpected Events
4. Minimize the Hazards
5. Use the Right Tool for the Job
6. Protect the Person
7. Use Procedures as Tools
8. Assess People's Abilities
9. Isolate the Equipment
10. Audit These Principles
Use the tag on equipment that is isolated often to remind the electrical worker of the simple Test-Before-Touch rules. Hand out the tag when an Energized Electrical Work Permit has been completed.
Contact your local printing company, they can provide you with the tags (make sure there is a hole punched out) and a strap so you can use the tag on equipment. You can also get these made up as magnets.
